Everyone Knew. Nobody Reported It.

Operational briefing note highlighting a culture where concerns are known but not raised or reported.

Sometimes the most revealing phrase after an incident isn't found in a report, is "Everybody knew."

The details circulate quickly, Teams speculate, stories spread and the bush telegraph works overtime. Yet when records are reviewed, there is often no formal concern raised, no incident logged, no investigation completed and no meaningful discussion recorded. Knowledge existed, learning didn't.

The issue isn’t what people knew. It's whether they felt able, encouraged or expected to do something with that knowledge. An organisation will never learn from incidents that officially never happened.
